What is the “true north” for the health care quality improvement movement? In December, Dr. Don Berwick took the podium at IHI’s National Forum and said that the quality movement has lost its way. Dr. Berwick, President Emeritus and Senior Fellow at IHI, called for a shift to a new era, a “moral era,” in which all health care work is based on the healing relationship between patient and provider. Health care, Dr. Manchanda says, has rescuers and raft builders to save people once they’re drowning, but it doesn’t have nearly enough people looking for the root causes to the problems. In this IHI Open School short, Dr. Manchanda creatively explains why he’s an “upstreamist” — and why the health care workforce is still swimming against the current. When medical student Nat Fondell first started seeing transgender patients at a specialty clinic, it was like walking on eggshells. “Even the most straightforward appointment felt like doing a delicate dance: I struggled to strike a balance between saying too little…and saying the wrong thing,” Fondell writes in Pulse. But with experience, providers at the specialty primary care clinic for transgender patients have learned that acceptance begets acceptance. It’s a lot harder at other health care organizations that are less familiar with transgender patients, The New York Times reports.
